The pre-admission process helps to provide the best possible care for you. Coming prepared to your appointment can improve your overall experience.

The Registration Process

The first step in the pre-admission process is registration. Your personal information is entered into our computer system, sign consent forms, and complete financial arrangements at that time.

Pre-Admission Services (PAS)

If you are scheduled for surgery at Ayers Surgery Center or Shands AGH, you will need to pre-register at Ayers Medical Plaza Suite 454 North Tower. Ayers Medical Plaza’s address is 720 SW 2nd Avenue, which is located across the street from Shands AGH.
To schedule your pre-operative appointment, please call 352.338.2113. Scheduling an appointment will expedite your PAS experience.
You will be seen by a registered nurse for a physical assessment and any diagnostic testing that your physician orders.
It is important to understand that the entire pre-admission process may take two to four hours.

Come Prepared

Please bring:

Photo Identification
Insurance Information
A current list of all medications
Your medical history
The name and phone number of your primary care physician
Any legal documents, such as guardianship, living wills and/or advanced directives

Day of Surgery

Check in at either Shands AGH Admissions Department if you are scheduled for surgery at Shands AGH, or at Ayers Surgery Center if you are scheduled for surgery at Ayers Surgery Center
Bring any applicable co-payment amount at that time
