measurement is rapidly emerging as a key component of the
Shands HealthCare Quality Initiatives. The Liver Transplant
Program is a leader in measuring, evaluating and reporting
clinical outcomes. Using variance data from the employment
of clinical pathways and clinical data generated by our
program, the team uses the results to identify and initiate
improvements in how they manage patient care. Additionally,
the team evaluates patient satisfaction and functional health
status.
The
Liver Transplant Team has performed more than 1,000 liver
transplants in patients with end-stage liver disease.
Since
December 2001, there were 819 transplants, 746 of these
were first grafts. The first graft recipients range in age
from 21 days to 68 years old:

120 (or 16%) have been 18 or younger
-
43 (or almost 6%) have been less than 12 months old
- 40 (or 5%) have been over 65
The
retransplant rates for adults and children are below their
respective national averages. Of the first 400 patients,
13 received both a liver and kidney, one has received a
liver and a pancreas and 386 received a liver alone.
In our
adult patients, the most common diagnoses before transplantation
are:
- Cirrhosis
from hepatitis C virus infection (almost 27%)
-
Cryptogenic cirrhosis (10%)
-
Alcoholic liver disease (10%)
-
Primary biliary cirrhosis (almost 6%)
- Cirrhosis
from hepatitis C/EtOH (13%)
Biliary
atresia is the most common diagnosis for our pediatric patients.
According
to the 1997 Report of Center Specific Graft and Patient
Survival Rates published by the United Network for Organ
Sharing (UNOS), the Shands Transplant Center Liver Transplant
Program had the best one-year survival rate of programs
in Florida between 5/1/92 and 4/30/94. Since then, our outcomes
have continued to improve, even though we are transplanting
patients who are sicker. For our most recent 200 patients,
our one-year survival rate is 88 percent. Long-term survival
in all patients now approaches 84 percent (two years and
beyond).
The
experience and innovation of our UF transplant surgeons
and other members of the Shands Transplant Center Liver
Transplant Team have resulted in a reduced amount of blood
transfused during liver transplant surgery, a reduced length
of stay, and reduced overall cost. In 1997, the University
HealthCare Consortium (UHC) reported that our Liver Transplant
Program's cost per discharge on 93 patients was significantly
lower than the mean cost reported by other programs. Recent
data from the Center for Healthcare Industry Performance
Studies confirmed that the Shands Transplant Center Liver
Transplant Program is the lowest-cost provider for liver
transplantation in Florida and the Southeast.
